Grade Level Physicals



Beginning July 1, 2018 all New York State school students must have a health exam when they enter school as a new entrant and in Pre-K or Kindergarten as well as grades 1, 3, 5, 7,9, and 11.  These examinations must be completed on the approved NYSED Student Health Examination Form for school.  This exam must include body mass index and weight status category information.

We understand that your medial providers office may not yet be aware of the changes so if you have already had a physical examination completed for the 2018-2019 school year on a different form the school may accept the physical on that form.  In 2019-2020 ONLY the approved form will be accepted.

You are encouraged to have your family physician examine your child and forward a copy of his/her physical exam to the Health Office.  Your child's doctor is more familiar with your child's health history and consequently can conduct a more comprehensive examination of your child that can be done at school.

If we do not receive a copy of your child's physical exam as required, he/she will be scheduled for an exam in school with the school physician.  You will be notified of any abnormal findings.  Dr. Tuttle-Malone is the schools Medical Director.  Her Physician Assistant, Melinda Rosner, will complete required physicals.  Physical forms may be faxed to: 315-346-6775 or 315-346-6005.
